Petit test
Guess first, no penalty. In a 2025 trial of AI scribes with 238 doctors, how often did reviewers find clinically significant mistakes in the notes?
An AI scribe listens to a visit and drafts the clinical note for the clinician to review and sign. In a randomized trial at UCLA, published in NEJM AI in 2025, one scribe cut time spent in notes by 9.5%, another made no significant difference, and clinically significant inaccuracies were noted "occasionally".
The clinician signs the note. But records staff often see what doesn't fit the rest of the chart: an MRI "ordered" with no order on file, a symptom the patient denied written as present, last year's problems copied into today. You don't change clinical content. You ask the author.
Trouvez les erreurs
The chart shows: an MRI discussed for later, not ordered. The patient denied chest pain. A physical therapy referral was sent. Tap the two lines that don't match.

Ask the author
Dr. Haddad, a question on your note for [patient ID], visit 3 Oct.
The note says "MRI lumbar spine ordered", but I can't find an MRI
order, and the plan says "consider MRI if no improvement in 4 weeks".
Could you check which is right and add an amendment if needed?
