Guess first, no penalty. An AI-drafted handover says "Patient stable, comfortable, mobile." What's the biggest risk with a line like that?
A handover (passing care to the next shift) is where risks travel or get lost. AI summaries sound calm, and that's the danger: a fall risk or new pain can disappear into "stable".
The NMC Code asks nurses to "identify any risks or problems that have arisen and the steps taken to deal with them, so that colleagues who use the records have all the information they need". A handover that drops the risk fails that test, however tidy it reads.
Many teams use SBAR to structure a handover: Situation, Background, Assessment, Recommendation. It puts the risk in the first line, where nobody can miss it.
A worked example
Your made-up notes from the shift: fall risk flagged at 12:30, the patient asked for pain relief at 14:00, needs one person's help to the bathroom.

Switch between the AI draft and the fixed handover. Highlights show what changed.
AI draft
Patient stable, comfortable, mobile. No concerns.
The draft was wrong three times: it hid the fall risk, denied the pain and claimed the patient walked alone.
When you ask AI to help, give it one rule it can't break: "Use only my notes. Put the main risk first. Write [missing] instead of guessing."
