Guess first, no penalty. This AI-drafted note is made up. What really happened: the patient reported new knee pain at 10:15 and you helped them to the bathroom. No pain relief yet, no family visit. Tap the three lines you can't sign.

AI-drafted note

0 sur 3 trouvées

When a detail is missing, AI drafting tools fill the gap with something that sounds right. In a nursing note, that can mean care that never happened. In a randomized trial of two AI scribes with 238 doctors at UCLA, the authors saw clinically significant inaccuracies "occasionally" and said they "require ongoing vigilance".

In the UK, the NMC Code (the professional rules for nurses) says records must be completed "accurately and without any falsification", with no "speculation". Outside the UK, check your own regulator's rules on records.

A worked example

Avant et après

Switch between the AI draft and the note the nurse signed.

AI draft

10:15 Patient reported new pain in the left knee. 10:20 Pain relief given, pain resolved. Walked to the bathroom with help from one nurse. Family visited and agreed with the plan. Likely developing an infection.

Three lines went. One true line was added. Every line left is something the nurse saw or did.

Your turn

Complétez les trous

Fill each gap to build a check you can run on any draft line.

My three questions

1. Did it ? 2. Is it the right ? 3. Is it ?

Question 1 sur 3

An AI-drafted note says "pain relief given at 14:00, pain resolved". You gave nothing at 14:00 and the patient is still in pain. What do you do with the note?
